New process: Risk stratification and patient need group (PNG) scores

A new way to help us care for you

At Fishermead Medical Centre we have introduced a new system to make sure you receive the right care from the right professional at the right time.

This gives every patient a Patient Need Group (PNG) score.

This helps us:

  • Know who might need more support.
  • Book you with the right person (doctor, nurse, pharmacist, or another team member).
  • Make sure you get safe and timely care.
  • Provide proactive care, helping identify who is at risk before they get more unwell.
  • Improve safety and ensure quicker access to the right support.

What this means for you

  • Clinicians may ask you a few more questions during your appointments
  • You may be contacted by our Care-Coordinators proactively to call you in for health checks
  • You might see your score displayed in your NHS record.
  • The PNG score is just a guide. A doctor or nurse will always decide what is best for your care pathway.
  • Your information is safe and follows NHS data privacy rules.

You don’t need to do anything — this will simply help us look after you better.
