Registering with the surgery
To register with the surgery:
- complete a NHS Register with a GP surgery form online
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
We request that after submitting this form you contact the surgery to make sure we have everything we need to complete the registration process. You may need to provide:
- Bank statement with current address, if you have recently moved into the property, any legal documentation with the address will be sufficient
- Photographic ID – Driving licence or passport
